In an effort to accommodate all of our patients, we require 24 hours advance notice in the event that you need to reschedule or cancel your appointment.
We are excited to meet you and begin our therapeutic alliance. We have a lot to accomplish during your visit so please follow these guidelines:
Arrive on time to your appointment. Please fill out all documents via the online intake paperwork that was emailed to you. Your initial visit will consist of a review of your health history, a thorough examination and treatment.
Wear appropriate clothing. It will be important for your sports chiropractor to have access to the injured area. Wear clothing that will allow your doctor access the area in need of treatment.
Please bring any imaging studies & radiology reports with you. This includes; x-rays, MRI, CT, scans & lab work.
Turquoise Trail Therapeutics is a fee-for-service clinic and we are considered an “out-of-network” provider for all insurance companies. In many instances we provide the patient with a “super bill,” detailing the diagnostic and treatment codes required by insurance companies in an effort to reimburse you directly. To find out about your out-of-network chiropractic benefits, call the phone number on the back of your insurance card.
Turquoise Trail Therapeutics offers treatment to help patients recover from injuries sustained during automobile accidents. We understand the frustration and stress that come along with car accidents. During your first visit we will outline a treatment plan that will work best for your specific case and work with your insurance company or attorney in an effort to help you recover fully.
Turquoise Trail Therapeutics offers treatment on work related injury. Dr. Casey has extensive experience helping patients recover from these types of injuries and will coordinate all aspects of workers compensation.
Examinations and treatments are payable at the time of service. We accept cash, check, Visa & Mastercard.
